DSD priorities and person-centered outcomes
DSD's published priorities cover community participation; health, wellness and safety; a home of one's choosing; enduring relationships; control of supports; and work and income. Its goals address access, timely service, quality, partnership, innovation and person-centered decisions. The introduction displays an old review date, so it does not establish current internal targets.

| Fictional expressed choice | “I want to keep seeing the people I know in my community.” |
|---|
| A question to understand it | “Which relationships and activities would you like the plan to support?” |
|---|
| Across partners | Carry that stated goal into planning, provider discussions and transition questions, with the person’s chosen supports. |

Three questions to keep separate| Question or relationship | What to consider |
|---|
| Licensure | Which current licensing requirements and processes apply? Check the licensing source. |
|---|
| MHCP enrollment | What does the enrollment source require of the provider? A license is not the same question. |
|---|
| Individual service planning | What needs to be addressed with the person’s lead agency? Do not infer authorization from licensure or enrollment. |
|---|
Verify the current exception criteria and process before advising a provider. Keep licensing, Medicaid enrollment and individual service authorization distinct.
