Fictional worked example

Whose goal appears in the plan?

Two invented planning notes concern the same person, who says they want to join a community writing group. Compare a service-centered entry with one that keeps the person's goal visible.

  1. Service-centered entry

    “Continue the available weekly activity.” The note names a service, but says little about the person's interest or what would make participation possible.

    Where is the person's own goal, and what choices were discussed?

  2. Goal-centered entry

    “Explore the writing group the person chose. Ask what communication, timing or travel support they want, compare available options together and agree a follow-up.”

    Who needs to confirm each support, and how will the person shape the next decision?
