Writing it down without stigma

An incident note is read by people who were not there, sometimes for years. A supervisor reads it to decide what happened. A new worker reads it before a first visit. A reviewer reads it when a complaint is filed. A person may read it themselves, or their advocate may. Each of those readers will take a label as a finding. “Aggressive” becomes a fact about the person. “Manipulative” becomes a reason not to believe them. “Noncompliant” becomes a reason to stop offering choices. The note you write in five minutes shapes every encounter that follows.

Objective documentation records four things. Behavior: what you saw and heard, in plain words, in order. Context: what was happening in the room and the process, including what had just been said to the person, the noise, the wait, who was present. Staff actions: what you and others did, including what you tried before anything else. Outcome: how it ended and what the person said or did afterward. Leave out motives you cannot know, diagnoses used as explanations, comparisons to other people, and any word that describes character rather than action.

This is not softening. A note that says “he stood, pushed a chair over with his foot so that it fell away from staff, and said loudly that no one was listening to him” is more precise and more useful than “he became aggressive.” It tells the reader exactly how serious it was, what preceded it, and what the person was communicating. It also happens to be fair.
